An MDS Coordinator (Minimum Data Set Coordinator) is a highly skilled nursing professional responsible for overseeing the comprehensive assessment process for residents in long-term care facilities. Utilizing the federally mandated MDS system, the Coordinator ensures precise documentation, facilitates effective care planning, and guarantees appropriate Medicare and Medicaid reimbursement. Key Responsibilities * Oversee MDS Assessments: Coordinate and ensure the timely and accurate completion of the Minimum Data Set (MDS) for all residents, maintaining compliance with applicable standards. * Interdisciplinary Collaboration: Partner effectively with nursing staff, physical and occupational therapists, dietitians, and attending physicians to collect comprehensive data and develop holistic, person-centered care plans. * Care Planning: Utilize MDS data to formulate individualized care plans that respectfully address each resident's unique strengths, needs, and preferences. * Reimbursement & Compliance: Ensure all documentation complies with federal regulations, including OBRA guidelines, to facilitate accurate Medicare and Medicaid reimbursements, incorporating RUGs classifications and facility quality measures. Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing
• Current state license as an RN
• RAC-CT(REsidnet Assessment Coordinator Certified) preferred but willing to train the right candidate.
• Proof of current CPR certification
• Compassionate, friendly demeanor
• Excellent communication skills
• Must be able to pass a background check and fingerprint process
