As a Hospice Registered Nurse (RN) - Per Diem, you will have the ability to provide meaningful end of life care where patients call home. Become an integral part of a dynamic team while maintaining a high level of autonomy. End of Life experience beneficial, although we will train the right individual.

Responsibilities for Hospice Registered Nurse (RN) - Per Diem:

• Complete home and facility visits during scheduled hours based upon patient and caregiver needs
• Manage comfort measures to ensure quality of life that is based upon the patient’s goals
• Maintain accurate, up to date documentation based on current CMS guidelines

Qualifications for Hospice Registered Nurse (RN) - Per Diem:

• Active NH RN license
• Minimum 1-2 years direct care experience required
• End of Life experience preferred
• Excellent communication skills necessary
• Proof of Flu vaccination
• Reliable transportation
• Valid driver's license
• Automobile insurance
• Physical exam within the last year
• Two-step TB test
