Family Nurse Practitioner (FNP)

Salary and Benefits

• Salary range of $109,000 to $152,000
• One-time $7,500 retention bonus at the start of the second year
• CME stipend of $2,000 and 40 hours of paid CME time per contract year
• 3 weeks of paid vacation annually, accrued throughout the year
• Paid sick leave
• 4-day work week (four 10-hour shifts, Monday–Thursday or Tuesday–Friday)
• No call responsibilities
• Relocation assistance available

• Comprehensive medical, dental, vision, life, short-term, and long-term disability insurance
  • Employee Only: Medical $30, Dental $0.50, Vision $0.50 – total $62.00/month
  • Employee + Spouse: Medical $155, Dental $15.90, Vision $3.92 – total $349.64/month
  • Employee + Child(ren): Medical $135, Dental $23.71, Vision $4.12 – total $325.66/month
  • Employee + Family: Medical $225, Dental $38.15, Vision $9.82 – total $545.94/month
  • Insurance begins the 1st of the month following your hire date
• Paid license, DEA, and professional membership fees
• Retirement Plan – 403(b) with 3% employer match when employee contributes 6% (9% total), effective the 1st of the month following hire
• Incentive bonuses may be available in the future

Responsibilities

• Provide primary care to patients of all ages in an outpatient setting
• Diagnose and treat acute and chronic conditions using evidence-based care
• Conduct physical exams and health screenings
• Prescribe medications and develop treatment plans
• Educate patients on preventive health practices and disease management
• Collaborate with multidisciplinary care teams to ensure continuity of care
• Document patient encounters in the EMR accurately and timely
• Participate in quality improvement initiatives and peer review processes
• May be required to provide coverage at nearby clinics if needed
• Support community outreach and education efforts

Requirements

• Board-certified Family Nurse Practitioner (FNP-C or FNP-BC)
• Current and unrestricted Arizona license or eligibility to obtain one
• Valid DEA registration
• BLS (CPR) certification
• Must meet credentialing standards for contracted health plans
• Willingness to work with underserved and culturally diverse populations
• Valid driver's license and reliable transportation
• Two years of ambulatory clinical experience preferred
• Multilingual skills in Navajo or Spanish are a plus
